Zemicef 400 mg Capsule

Indication:

Zemicef 400 mg Capsule is indicated for the treatment of-

• Urinary Tract Infections (UTI): Uncomplicated infections caused by E. coli and P. mirabilis.
• Respiratory Tract Infections: Including acute bronchitis, acute exacerbations of chronic bronchitis, and pneumonia.
• Otitis Media: Middle ear infections in children caused by H. influenzae or S. pyogenes.
• Pharyngitis and Tonsillitis: Caused by S. pyogenes.
• Uncomplicated Gonorrhoea: Urethral or cervical infections caused by N. gonorrhoeae.
• Typhoid Fever: Often used in cases of multi-drug resistant enteric fever.

Dogase & Adminstration:

Adults (Oral):

• General Infections: 200–400 mg daily as a single dose or in two divided doses.
• Uncomplicated Gonorrhoea: A single dose of 400 mg.

Children (Oral – Based on BNFC):

• 6 months to 10 years (Weight up to 50kg): 8 mg/kg daily as a single dose or in two divided doses.
• Over 10 years (or >50kg): 200–400 mg daily (Adult dose).

Administration: Can be taken with or without food. Taking it with food may improve gastrointestinal tolerance. The oral suspension should be shaken well before use.

Pharmacology:

• Mechanism of Action: Cefixime is a bactericidal agent. It acts by inhibiting bacterial cell wall synthesis. It binds to specific penicillin-binding proteins (PBPs) located inside the bacterial cell wall, which inhibits the final transpeptidation step of peptidoglycan synthesis. This leads to cell wall lysis and death of the bacteria.
• Pharmacodynamics: It is highly stable in the presence of beta-lactamase enzymes. This stability allows it to be effective against many organisms that are resistant to penicillins and some other cephalosporins.
• Pharmacokinetics: About 40% to 50% is absorbed after oral administration. It has a relatively long half-life (about 3–4 hours), which allows for once-daily or twice-daily dosing. It is excreted primarily unchanged in the urine.

Side Effects:

• Common: Diarrhea (frequent), nausea, abdominal pain, and dyspepsia.
• Hypersensitivity: Skin rashes, urticaria, and rarely, anaphylaxis.
• Others: Headache, dizziness, and genital pruritus.

Drug Interaction:

• Warfarin: May increase prothrombin time (risk of bleeding).
• Carbamazepine: Cefixime may increase the plasma concentration of carbamazepine.
• Probenecid: May slow the renal excretion of cefixime.

Pregnancy & Lactation:

• Pregnancy: Generally considered safe (Category B), but should only be used if clearly needed.
• Lactation: It is not known if cefixime is excreted in human milk. Discontinuing breastfeeding temporarily during therapy may be considered.

Over Dosage:

No specific symptoms of overdose are known. In case of massive ingestion, gastric lavage may be indicated. It is not significantly removed by dialysis.

Therapeutic Class:

Third-generation Cephalosporin; Beta-lactam Antibiotic.

Storage Conditions:

Store at room temperature (below 30°C), protected from light and moisture. After reconstitution, the oral suspension is usually stable for 14 days at room temperature or in a refrigerator.
