Voniza 10 mg

Indication:

Voniza 10 mg is indicated for the treatment of-

• Gastro-oesophageal Reflux Disease (GORD): Treatment and prevention of recurrence of erosive oesophagitis.
• Peptic Ulcers: Treatment of gastric and duodenal ulcers.
• H. pylori Eradication: Used as part of a triple or dual therapy regimen with antibiotics (Amoxicillin/Clarithromycin).

Dogase & Adminstration:

Adults (Oral):

• Erosive Oesophagitis: 20 mg once daily for 4–8 weeks.
• GORD Maintenance: 10 mg once daily for long-term management.
• Gastric/Duodenal Ulcers: 20 mg once daily for 6–8 weeks.
• H. pylori Eradication: 20 mg twice daily for 7–14 days (combined with antibiotics).

Children (Oral – Based on BNFC/Specialist guidance):

• Note: Safety and dosage for pediatric patients are currently under investigation. Usage is typically restricted to adolescents (12–18 years) at adult doses or as specifically directed by a pediatric gastroenterologist.

Administration: Can be taken with or without food. Tablets should be swallowed whole with water.

Pharmacology:

• Mechanism of Action: Vonoprazan inhibits gastric H+/K+-ATPase in a reversible and potassium-competitive manner. Unlike traditional PPIs (which require acid activation and bind covalently), Vonoprazan does not require acid activation and binds directly to the potassium-binding site of the proton pump.
• Pharmacodynamics: It provides a faster onset of action and achieves a more stable and higher pH level in the stomach from the first dose. It remains stable in acidic environments, allowing for sustained acid suppression both day and night.
• Pharmacokinetics: It is rapidly absorbed with high bioavailability. It has a long half-life, providing sustained inhibition. It is primarily metabolized in the liver by the CYP3A4 enzyme system.

Side Effects:

• Common: Diarrhea, constipation, nausea, abdominal bloating, and rash.
• Others: Increased levels of gastrin in the blood (hypergastrinaemia).

Drug Interaction:

• Since it increases gastric pH, it may affect the absorption of pH-dependent drugs (e.g., Ketoconazole, Atazanavir).
• Potential interactions with drugs metabolized by CYP3A4 (though clinically significant interactions are less frequent than with some PPIs).

Pregnancy & Lactation:

• Pregnancy: Limited data available. Use is generally not recommended unless the benefit outweighs the potential risk.
• Lactation: It is unknown if it is excreted in breast milk. Caution is advised.

Over Dosage:

No specific antidote is available. Management is symptomatic and supportive.

Therapeutic Class:

Potassium-Competitive Acid Blocker (P-CAB); Antisecretory agent.

Storage Conditions:

Store below at 30°C  temperature in a dry place. Away from light and out of reach of children. Do not use the medication after its expiration date.
