Ace 60 ml Suspension

Indication:

Ace 60 ml Suspension is indicated for the treatment of-

• Mild to Moderate Pain: Headache, migraine, tension headache, toothache, backache, and rheumatic/muscular pain.
• Pyrexia (Fever): Reduction of fever including post-vaccination febrile reactions.
• Post-operative Pain: Management of pain following surgical procedures (often via IV).

Dogase & Adminstration:

Adults (Oral):

• 500 mg to 1 g every 4–6 hours as required.
• Maximum Dose: 4 g (4000 mg) in 24 hours.

Children (Oral – Based on BNFC 2022-2023):

• 3 months to 1 year: 60 mg–120 mg every 4–6 hours.
• 1 to 6 years: 120 mg–250 mg every 4–6 hours.
• 6 to 12 years: 250 mg–500 mg every 4–6 hours.
• Over 12 years: 500 mg–1 g every 4–6 hours.

Rectal Suppository (Weight-Based – BNFC):

• Dose: 10–15 mg/kg every 4–6 hours.
• Maximum: 60 mg/kg daily in divided doses.

Intravenous Injection (Weight-Based – BNF):

• Patients >50 kg: 1 g every 4–6 hours (Max 4 g daily).
• Patients 33 kg to 50 kg: 15 mg/kg every 4–6 hours (Max 60 mg/kg or 3 g daily).
• Neonates & Infants (<33 kg): 7.5 mg/kg to 10 mg/kg per dose.

Pharmacology:

• Mechanism of Action: Paracetamol produces analgesia by inhibiting prostaglandin synthesis in the Central Nervous System (CNS) and to a lesser extent through a peripheral action by blocking pain-impulse generation. Its antipyretic effect is due to its action on the hypothalamic heat-regulating center, leading to vasodilation and increased peripheral blood flow, which dissipates body heat.
• Pharmacodynamics: Unlike NSAIDs, Paracetamol does not possess significant anti-inflammatory properties and does not inhibit peripheral COX-1 or COX-2 significantly, which is why it has minimal gastric irritation.
• Pharmacokinetics: It is rapidly absorbed from the gastrointestinal tract. Peak plasma concentrations occur within 30 to 60 minutes for tablets. It is metabolized primarily in the liver through conjugation and excreted via urine.

Side Effects:

• Rare: Skin rash, blood disorders (thrombocytopenia), and acute pancreatitis.

Drug Interaction:

• Warfarin: Prolonged regular use may enhance the anticoagulant effect.
• Metoclopramide: May increase the rate of absorption of paracetamol.

Pregnancy & Lactation:

• Pregnancy: Considered safe; however, the lowest effective dose should be used for the hortest time.
• Lactation: Excreted in breast milk in small amounts, but not known to be harmful to infants.s

Over Dosage:

• Warning: Paracetamol overdose can cause severe hepatic necrosis (liver failure). Symptoms may not appear for 24–48 hours.
• Antidote: N-acetylcysteine.

Therapeutic Class:

Analgesic and Antipyretic.

Storage Conditions:

Store below at 30°C  temperature in a dry place. Away from light and out of reach of children. Do not use the medication after its expiration date.
