Acedol

Indication:

Acedol indicated for the treatment of-

• Osteoarthritis: For the relief of pain and inflammation in degenerative joint disease.
• Rheumatoid Arthritis: Long-term management of autoimmune joint inflammation.
• Ankylosing Spondylitis: Treatment of chronic inflammatory arthritis affecting the spine.
• Acute Pain: Management of dental pain, post-traumatic pain, and lower back pain (Lumbago).

Dogase & Adminstration:

Adults (Oral):

• Standard Dose: 100 mg twice daily (morning and evening).
• SR Formulation: 200 mg once daily.

Children (Based on BNFC):

• Aceclofenac is not recommended for children as its safety and efficacy have not been established for the pediatric population in the provided documents.

Special Populations: In patients with hepatic impairment, the initial dose should be reduced to 100 mg daily.

Administration: Tablets must be swallowed whole with a glass of water. It is strictly recommended to take Aceclofenac during or after meals to minimize gastrointestinal side effects.

Pharmacology:

• Mechanism of Action: Aceclofenac is a phenylacetic acid derivative that acts as a potent inhibitor of the enzyme cyclo-oxygenase (COX), which is involved in the production of prostaglandins. Prostaglandins are the primary mediators of pain, inflammation, and fever. By inhibiting COX, Aceclofenac effectively reduces inflammatory swelling and pain intensity.
• Pharmacodynamics: It possesses high anti-inflammatory and analgesic properties. It has been shown to inhibit the synthesis of several inflammatory cytokines like Interleukin-1 and TNF, which are crucial in the progression of joint destruction.
• Pharmacokinetics: It is rapidly and completely absorbed after oral administration. Peak plasma concentrations are reached in 1.25 to 3 hours. It is highly protein-bound (>99%) and is eliminated primarily through urine as hydroxy metabolites.

Side Effects:

• Common: Dizziness, dyspepsia (indigestion), abdominal pain, nausea, and diarrhea.
• Rare: Peptic ulceration, gastrointestinal bleeding, and skin rashes.

Drug Interaction:

• Lithium/Digoxin: May increase plasma concentrations of these drugs.
• Anticoagulants (Warfarin): Increases the risk of bleeding.
• Diuretics: May reduce the antihypertensive effect of diuretics.
• Other NSAIDs: Increases the risk of gastrointestinal ulcers.

Pregnancy & Lactation:

• Pregnancy: Should be avoided, especially during the third trimester, as it may cause premature closure of the fetal ductus arteriosus.
• Lactation: Use is not recommended unless the benefits outweigh the risks; it is unknown if it is excreted in breast milk.

Over Dosage:

Symptoms include severe headache, nausea, vomiting, and epigastric pain. Management involves gastric lavage and supportive therapy.

Therapeutic Class:

Non-steroidal Anti-inflammatory Drug (NSAID).

Storage Conditions:

Store below at 30°C  temperature in a dry place. Away from light and out of reach of children. Do not use the medication after its expiration date.
