Indication:

Adegut 60 ml is indicated for the treatment of-

• Nausea and Vomiting: Relief of symptoms of nausea and vomiting of various origins.
• Dyspeptic Symptom Complex: Treatment of epigastric fullness, early satiety, abdominal bloating, and upper abdominal pain.
• Gastro-oesophageal Reflux Disease (GORD): To reduce symptoms like heartburn and regurgitation.
• Diabetic Gastroparesis: Management of delayed gastric emptying in diabetic patients.

Dogase & Adminstration:

Adults & Adolescents (Over 35 kg):

• Oral: 10 mg up to three times daily.
• Suppository (Rectal): 30 mg to 60 mg twice daily.

Children (Based on BNFC 2022-2023):

• Oral (Weight-based): 0.25 mg/kg per dose, administered up to three times daily.
• Suppository (Weight-based): For children over 15 kg, the dose is approximately 0.75 mg/kg to 1 mg/kg twice daily.

Maximum Duration: Treatment should not usually exceed 7 days.

Administration: It is strongly recommended to take Domperidone 15–30 minutes before meals. Taking it after meals may delay absorption.

Pharmacology:

• Mechanism of Action: Domperidone is a peripheral dopamine receptor antagonist. It acts primarily at the Chemoreceptor Trigger Zone (CTZ) located outside the blood-brain barrier and on the gastrointestinal tract. By blocking dopamine receptors, it inhibits the relaxation of the stomach muscles and prevents the transmission of nausea signals to the brain.
• Pro-kinetic Activity: It increases the duration of antral and duodenal contractions and accelerates gastric emptying. It also increases lower esophageal sphincter pressure, which prevents the backflow of stomach acid (reflux).
• Pharmacodynamics: Because it does not easily cross the blood-brain barrier, it has a very low incidence of central nervous system side effects (like tremors) compared to other dopamine antagonists.
• Pharmacokinetics: It is rapidly absorbed after oral administration, reaching peak plasma levels within 30 to 60 minutes. It is extensively metabolized in the liver and excreted through feces and urine.

Side Effects:

• Common: Dry mouth, headache, and diarrhea.
• Hormonal: Increased prolactin levels, which may lead to breast enlargement (gynecomastia) or nipple discharge.

Drug Interaction:

• Anticholinergics: May antagonize the pro-kinetic effect of Domperidone.
• Ketoconazole/Erythromycin: May increase blood levels of Domperidone, increasing the risk of heart rhythm problems.

Pregnancy & Lactation:

• Pregnancy: Use only when strictly necessary and prescribed by a doctor.
• Lactation: Small amounts are excreted in breast milk; generally considered safe but monitoring the infant for side effects is advised.

Over Dosage:

Symptoms include agitation, altered consciousness, and convulsions. Management involves gastric lavage and administration of activated charcoal.

Therapeutic Class:

Dopamine Antagonist; Anti-emetic and Pro-kinetic.

Storage Conditions:

Store below at 30°C  temperature in a dry place. Away from light and out of reach of children. Do not use the medication after its expiration date.
