Aeron 10 mg

Indication:

Aeron 10 mg is indicated for the treatment of-

• Prophylaxis and Chronic Treatment of Asthma: Used for the long-term prevention of asthma symptoms in adults and children.
• Exercise-Induced Bronchoconstriction (EIB): Prevention of narrowing of the airways triggered by exercise.
• Seasonal Allergic Rhinitis: Relief of symptoms such as sneezing, nasal congestion, and runny nose (primarily for patients 2 years and older).
• Perennial Allergic Rhinitis: Management of year-round allergy symptoms in adults and pediatric patients at least 6 months of age.

Dogase & Adminstration:

Adults (15 years and older): One 10 mg tablet daily.

Pediatric Dosage (Based on BNFC 2022-2023):

• 6 months to 5 years: 4 mg once daily (Granules or Chewable tablet).
• 6 years to 14 years: 5 mg once daily (Chewable tablet).
• 15 years and older: 10 mg once daily.

Note on Granules: Can be administered directly in the mouth or mixed with a spoonful of cold/room temperature soft food (e.g., applesauce, ice cream). Do not dissolve in liquid.

Administration: Montelukast should be taken once daily in the evening for asthma or allergic rhinitis. For patients with both, one evening dose is sufficient.

Pharmacology:

• Mechanism of Action: Montelukast is a selective and potent cysteinyl leukotriene receptor antagonist. Cysteinyl leukotrienes ($LTC_{4}$, $LTD_{4}$, $LTE_{4}$) are powerful inflammatory mediators released from various cells, including mast cells and eosinophils. These leukotrienes bind to cysteinyl leukotriene (CysLT) receptors found in the human airway, causing bronchoconstriction, mucous secretion, vascular permeability, and eosinophil recruitment.
• Pharmacodynamics: Montelukast binds with high affinity and selectivity to the CysLT1 receptor, inhibiting the physiological actions of $LTD_{4}$ without any agonist activity. This results in reduced airway edema, smooth muscle contraction, and cellular inflammatory activity.
• Pharmacokinetics: It is rapidly absorbed after oral administration. For the 10 mg tablet, the mean peak plasma concentration ($C_{max}$) is achieved in 3 hours. It is over 99% bound to plasma proteins. It is extensively metabolized in the liver and excreted almost exclusively via bile into the feces.

Side Effects:

• Common: Headache, abdominal pain, thirst, and hyperkinesia (in children).
• Neuropsychiatric Events: Rare but serious events including agitation, aggression, dream abnormalities, and suicidal ideation.

Drug Interaction:

• Phenobarbital: May decrease the plasma concentration of Montelukast due to hepatic metabolism induction.
• Note: No dosage adjustment is generally required when co-administered with theophylline, prednisolone, or oral contraceptives.

Pregnancy & Lactation:

• Pregnancy: Clinical data is limited. Use only if clearly needed and prescribed by a doctor.
• Lactation: It is unknown if it is excreted in human milk; breastfeeding is generally not recommended during use.

Over Dosage:

Symptoms of acute overdose include abdominal pain, somnolence, thirst, headache, vomiting, and psychomotor hyperactivity. Treatment is supportive.

Therapeutic Class:

Leukotriene Receptor Antagonist (LTRA).

Storage Conditions:

Store below at 30°C  temperature in a dry place. Away from light and out of reach of children. Do not use the medication after its expiration date.
