Finix 20 Tablet

Indication:

Finix 20 mg tablet is indicated for the treatment of-

• Active Duodenal Ulcer: Treatment of acute duodenal ulcers.
• Active Benign Gastric Ulcer: Treatment of active benign gastric ulcers.
• Gastro-oesophageal Reflux Disease (GORD): Treatment of symptomatic erosive or ulcerative GORD and long-term maintenance.
• Zollinger-Ellison Syndrome: Management of hypersecretory conditions.

Dogase & Adminstration:

Adults (Oral):

• Active Duodenal & Benign Gastric Ulcers: 20 mg once daily in the morning for 4–8 weeks.
• Erosive/Ulcerative GORD: 20 mg once daily for 4–8 weeks; followed by maintenance of 10–20 mg daily.
• Symptomatic GORD (without oesophagitis): 10 mg once daily for up to 4 weeks.
• Zollinger-Ellison Syndrome: Initially 60 mg once daily; doses up to 120 mg daily may be given if required.

Children (Oral – Based on BNFC):

• 12–18 years: Use adult dose (20 mg once daily).
• Note: Safety and efficacy for children under 12 years are not as extensively established in some regions; follow specific specialist guidance.

Administration: Tablets should be swallowed whole and taken in the morning before eating. Do not crush or chew.

Injection Dosage For Adults:

• Standard Dosage: 20 mg once daily administered via slow intravenous injection or infusion.
• Severe Conditions: In cases of severe acid-related symptoms, the dose may be increased up to 20 mg twice daily based on clinical judgment.
• Zollinger-Ellison Syndrome: Initial dose of 60 mg/day IV, which may be adjusted up to 120 mg/day in divided doses.

Injection Dosage For Children (Based on Clinical Practice & BNFC Context):

• Note: Safety and efficacy for children under 12 years are not as extensively documented as other PPIs.
• 12 to 18 years: Follow the adult dose of 20 mg once daily.

Administration Methods:

• IV Injection: Reconstitute the 20 mg vial with 5 mL of Sterile Water for Injection or Normal Saline. Inject slowly over 5 minutes.
• IV Infusion: Dilute the reconstituted 20 mg solution in 100 mL of Normal Saline or 5% Dextrose. Infuse over 15 to 30 minutes.

Pharmacology:

• Mechanism of Action: Rabeprazole belongs to the class of antisecretory compounds (substituted benzimidazoles). It suppresses gastric acid secretion by inhibiting the H+/K+-ATPase enzyme (the gastric acid pump) at the secretory surface of the parietal cell. It blocks the final step of acid secretion.
• Pharmacodynamics: After oral administration of a 20 mg dose, the antisecretory effect begins within one hour. Rabeprazole exhibits a more rapid activation over a wider pH range compared to other PPIs like omeprazole.
• Pharmacokinetics: It is acid-labile and absorbed in the small intestine. It is approximately 96.3% bound to human plasma proteins. Metabolism is primarily non-enzymatic with a minor part through the hepatic cytochrome P450 system (CYP2C19 and CYP3A4).

Side Effects:

• Common: Cough, sore throat, rhinitis, headache, dizziness, nausea, and flatulence.
• Rare: Erythema, bullous reactions, and interstitial nephritis.

Drug Interaction:

• Causes profound and long-lasting inhibition of gastric acid; hence, it may interfere with the absorption of drugs where gastric pH is critical (e.g., Ketoconazole, Digoxin).
• No clinically significant interactions with Warfarin, Phenytoin, or Diazepam are typically reported due to its non-enzymatic metabolism path.

Pregnancy & Lactation:

• Pregnancy: Avoid use during pregnancy. There are no data on the safety of rabeprazole in pregnant women.
• Lactation: It is unknown if it is excreted in breast milk. Should not be used during breastfeeding.

Over Dosage:

Experience with deliberate overdose is limited. No specific symptoms have been identified. Management is supportive.

Therapeutic Class:

Proton Pump Inhibitor (PPI); Anti-ulcerant.

Storage Conditions:

Store below at 30°C  temperature in a dry place. Away from light and out of reach of children. Do not use the medication after its expiration date.
