Jakrif 5 mg

Indication:

Jakrif 5 mg is indicated for the treatment of-

• Rheumatoid Arthritis (RA): For adults with moderately to severely active RA who have had an inadequate response to methotrexate.

• Psoriatic Arthritis (PsA): For adults with active PsA who have responded poorly to other DMARDs.

• Ulcerative Colitis (UC): For adults with moderately to severely active UC.

• Polyarticular Juvenile Idiopathic Arthritis (pcJIA): For patients 2 years of age and older.

Dogase & Adminstration:

• Administration: Can be taken with or without food. Tablets must be swallowed whole.
• Rheumatoid Arthritis: 5 mg twice daily or 11 mg once daily (extended-release).
• Psoriatic Arthritis: 5 mg twice daily in combination with non-biologic DMARDs.
• Ulcerative Colitis:
  • Induction: 10 mg twice daily for 8 to 16 weeks.
  • Maintenance: 5 mg twice daily; 10 mg twice daily may be considered for some.
• Pediatric (pcJIA): Dosage is weight-based (typically 5 mg twice daily for patients ≥40 kg).

Pharmacology:

Tofacitinib is a selective inhibitor of the Janus kinase (JAK) family of enzymes (specifically JAK1 and JAK3). In the human body, cytokines play a major role in signaling inflammation. These cytokines bind to receptors that use the JAK pathway to transmit signals to the nucleus. By inhibiting JAKs, Tofacitinib prevents the phosphorylation and activation of STATs (Signal Transducers and Activators of Transcription), effectively “turning off” the production of inflammatory mediators. This suppresses the overactive immune response characteristic of autoimmune diseases.

Side Effects:

Upper respiratory tract infections.Headache, diarrhea, and nasopharyngitis.Serious risks: Serious infections (TB, fungal), malignancies, and thrombosis.

Drug Interaction :

• CYP3A4 Inhibitors: Drugs like ketoconazole may increase Tofacitinib levels.
• CYP3A4 Inducers: Drugs like rifampin may decrease its efficacy.
• Immunosuppressants: Risk of additive immunosuppression when used with biologics or potent immunosuppressants.

Pregnancy & Lactation:

Pregnancy: Limited human data; use only if the benefit outweighs the risk to the fetus.Lactation: Breastfeeding is not recommended during treatment.

Over Dosage:

In case of overdose, provide supportive treatment. There is no specific antidote.
