Respira 2 mg Tablet

Indication:

Respira 2 mg Tablet is indicated for the treatment of-

• Acute Bronchospasm: Treatment or prevention of bronchospasm in patients with reversible obstructive airway disease (Asthma).
• COPD Management: Relief of airway obstruction in patients with chronic bronchitis or emphysema.
• Exercise-Induced Asthma: Prevention of wheezing and chest tightness triggered by physical exertion.
• Nighttime Symptoms: Management of nocturnal asthma symptoms to improve sleep quality.

Dogase & Adminstration:

Adults and Adolescents (12+ years):

• Oral: 1 mg to 2 mg three times daily.
• Inhalation: 1 to 2 puffs (50–100 mcg) every 4 to 6 hours as needed.
• Nebulization: 0.63 mg to 1.25 mg three times daily.

Children (Based on BNFC context):

• 6–11 years: 0.5 mg to 1 mg orally three times daily.
• Nebulization (6–11 years): 0.31 mg three times daily.

Note on Weight-Based Injection/Suppository: Levosalbutamol is not standardly available in injection or suppository forms in the primary British National Formulary context; intravenous treatment is usually reserved for racemic salbutamol.

Administration: For oral forms, take with or without food. For inhalers, use with a spacer if needed to ensure maximum lung delivery.

Pharmacology:

• Mechanism of Action: Levosalbutamol is the single functional (R)-isomer of the racemic mixture salbutamol. It acts as a selective Beta2-adrenergic agonist. By binding to Beta2-receptors on bronchial smooth muscle, it activates adenyl cyclase, increasing intracellular cyclic AMP (cAMP). This process leads to the activation of protein kinase A, which inhibits myosin phosphorylation and lowers intracellular calcium, resulting in profound bronchial relaxation. Unlike the (S)-isomer found in regular salbutamol, Levosalbutamol avoids pro-inflammatory pathways, providing a “cleaner” therapeutic effect.
• Pharmacodynamics: It provides rapid bronchodilation typically within 5–10 minutes when inhaled. It significantly reduces airway resistance and is less likely to stimulate Beta1-receptors in the heart, resulting in fewer cardiac side effects like tachycardia compared to the racemic version.
• Pharmacokinetics: Inhaled Levosalbutamol acts locally. Oral doses are absorbed and metabolized in the liver, primarily into levalbuterol 4′-O-sulfate, and excreted through the urine.

Side Effects:

• Common: Mild tremors, headache, and nervousness.
• Others: Dizziness, dry mouth, and rare instances of palpitations.

Drug Interaction:

• Non-selective Beta-blockers: May inhibit the effect of Levosalbutamol and cause severe bronchospasm.
• Diuretics: May increase the risk of hypokalemia (low potassium).
• Digoxin: May decrease serum digoxin levels.

Pregnancy & Lactation:

• Pregnancy: Use only if the benefit to the mother outweighs the potential risk to the fetus (Category C).
• Lactation: It is unknown if it is excreted in human milk; use with caution.

Over Dosage:

Symptoms include tremors, chest pain, and severe tachycardia. Management involves cardiac monitoring and correction of electrolyte imbalances.

Therapeutic Class:

Selective Beta2-Adrenoceptor Agonist; Short-Acting Bronchodilator.

Storage Conditions:

Store below at 30°C  temperature in a dry place. Away from light and out of reach of children. Do not use the medication after its expiration date.
