Saltamol 100 ml

Indication:

Saltamol 100 ml is indicated for the treatment of-

• Acute Bronchospasm: Rapid relief of airway narrowing in asthma, chronic bronchitis, and emphysema.
• Asthma Management: Treatment and prophylaxis of bronchial asthma.
• Exercise-Induced Bronchospasm: Prevention of asthma symptoms triggered by physical activity.
• Chronic Obstructive Pulmonary Disease (COPD): For reversible airways obstruction.
• Severe Acute Asthma (Status Asthmaticus): Management of life-threatening asthma (usually via nebulization or IV).

Dogase & Adminstration:

Inhalation (Aerosol):

• Adults: 100–200 micrograms (1–2 puffs) for symptom relief. For prophylaxis, 200 micrograms before exercise.
• Children: 100 micrograms (1 puff), increased to 200 micrograms if necessary.

Oral (Tablets/Syrup):

• Adults: 2–4 mg 3 to 4 times daily.
• Children (2–6 years): 1–2 mg (2.5–5 ml syrup) 3 to 4 times daily.
• Children (6–12 years): 2 mg (5 ml syrup) 3 to 4 times daily.

Intravenous Injection (Weight-Based):

• Adults: 250 micrograms (4 micrograms/kg) injected slowly.
• Children: 5 micrograms/kg as a single dose, adjusted according to response.

Note on Suppository: Salbutamol is not standardly available in suppository form in the primary referenced clinical documents.

Pharmacology:

• Mechanism of Action: Salbutamol is a highly selective beta2-adrenoceptor agonist. It acts primarily on the beta2-adrenergic receptors located on the bronchial smooth muscles. Binding to these receptors activates adenyl cyclase, which increases intracellular cyclic AMP. This increase in cAMP leads to the activation of protein kinase A, which inhibits the phosphorylation of myosin and lowers intracellular ionic calcium concentrations, resulting in muscle relaxation.
• Pharmacodynamics: It provides rapid bronchodilation (within 5 minutes via inhalation). It also inhibits the release of bronchoconstricting mediators from mast cells, such as histamine and leukotrienes. Unlike non-selective agonists, Salbutamol has minimal effect on the beta1-receptors of the heart at therapeutic doses.
• Pharmacokinetics: When inhaled, it acts locally on bronchial smooth muscle. Oral doses are well absorbed from the GI tract but undergo significant first-pass metabolism in the liver. It is primarily excreted in the urine.

Side Effects:

• Common: Fine tremors (usually in hands), nervous tension, headache, and palpitations.
• Rare: Muscle cramps and tachycardia.

Drug Interaction:

• Beta-blockers (e.g., Propranolol): May antagonize the effect of Salbutamol and cause severe bronchospasm.
• Diuretics: May increase the risk of hypokalaemia (low potassium).

Pregnancy & Lactation:

• Pregnancy: Use only if the potential benefit outweighs the risk. It is often used to manage asthma in pregnancy as uncontrolled asthma is more dangerous.
• Lactation: Generally considered safe as it is secreted in breast milk in very small amounts.

Over Dosage:

Symptoms: Tachycardia, CNS stimulation, tremor, and hypokalaemia. Treatment is symptomatic, focusing on cardiac monitoring and potassium levels.

Therapeutic Class:

Selective Beta2-Adrenoceptor Agonist; Bronchodilator.

Storage Conditions:

Store below at 30°C  temperature in a dry place. Away from light and out of reach of children. Do not use the medication after its expiration date.
