Faborsyn Capsule

Indication:

Faborsyn Capsule is indicated for the treatment of-

• Prophylaxis of Migraine Headaches: Indicated strictly for reducing the frequency, severity, and duration of classical and common migraine attacks, as well as associated symptoms like nausea, vomiting, photophobia (light sensitivity), and phonophobia (sound sensitivity).

  Crucial Note: It is not indicated for acute abortive therapy of an ongoing migraine attack; it is purely a long-term preventive agent.

• Chronic Tension Headaches: Preventive treatment for patients suffering from frequent tension-type headaches caused by muscle contractions or stress.

• Adjuvant in Mild Inflammatory Conditions: Used as a complementary therapy to alleviate chronic inflammatory symptoms related to mild osteoarthritis or rheumatoid arthritis due to its anti-NF-κB pathways.

Dogase & Adminstration:

1 Capsule 2 times daily or as per directed bythe register physician

Pharmacology:

The clinical efficacy of this formulation relies on the synergistic biological activities of its active chemical moieties, primarily sesquiterpene lactones (e.g., Parthenolide) combined with flavone derivatives.

• Inhibition of Pro-inflammatory Cascades (Anti-NF-κB Pathway): Parthenolide acts as a potent inhibitor of the IκB Kinase (IKK) complex. By inhibiting IKKβ, it prevents the phosphorylation and subsequent degradation of IκB, thereby arresting the nuclear translocation of Nuclear Factor-kappa B (NF-κB). This stops the genetic transcription of genes regulating pro-inflammatory cytokines such as Interleukin-1 (IL-1), Interleukin-6 (IL-6), and Tumor Necrosis Factor-alpha (TNF-α).

• Prostaglandin Synthesis Suppression: The extract suppresses the generation of arachidonic acid metabolites by downregulating cyclooxygenase-2 (COX-2) expression and phospholipase $A_2$ ($PLA_2$) activity, effectively halting the production of inflammatory prostaglandins.

• Serotonergic Antagonism (5-HT Receptor Inhibition): It acts similarly to a mild serotonin (5-HT) receptor antagonist. It prevents the abnormal release of serotonin and histamine from blood platelets (thrombocytes), which is a core physiological trigger for the vasoconstriction and subsequent rebound vasodilation seen in classical migraine attacks.

• Vascular Smooth Muscle Stabilization: It neutralizes endogenous vasoconstrictors, maintaining regular tone in the cerebral vasculature and mitigating neurogenic inflammation within the meninges.

Pharmacokinetics (PK)

• Absorption: Parthenolide exhibits moderate to rapid gastrointestinal absorption following oral administration. Peak plasma concentration ($C_{max}$) is typically achieved within 1 to 2 hours.

• Distribution: Lipophilic components readily distribute throughout peripheral tissues. Plasma protein binding ranges between 60%–70%.

• Metabolism: Extensively metabolized in the liver via Phase I hepatic cytochrome P450 enzyme pathways and subsequent Phase II conjugation.

• Elimination: Excreted primarily via the kidneys as metabolites and through biliary pathways. The elimination half-life ($t_{½}$) ranges from 3 to 6 hours.

Side Effects:

• Gastrointestinal: Abdominal pain, flatulence, bloating, heartburn, nausea, diarrhea, or mild constipation.

• Oral Manifestations: Chewing the dosage form causes mouth ulcers, glossitis, and temporary loss of taste.

• Post-Feverfew Syndrome: Abrupt discontinuation after long-term use can precipitate a rebound effect, characterized by severe headaches, anxiety, insomnia, rebound nervousness, and joint/muscle stiffness

Drug Interaction:

• Anticoagulants & Antiplatelets (e.g., Warfarin, Aspirin, Clopidogrel): Feverfew inhibits platelet aggregation. Concomitant use significantly increases the risk of bleeding and bruising. Monitor INR closely.

• NSAIDs (e.g., Ibuprofen, Naproxen): May partially blunt the long-term prophylactic efficacy of Feverfew or increase the risk of gastric mucosal irritation.

• Hepatic CYP450 Substrates: Feverfew inhibits certain liver enzymes (e.g., CYP3A4). Co-administration may increase plasma levels of drugs metabolized by these enzymes.

Pregnancy & Lactation:

• Pregnancy: Strictly contraindicated. Feverfew possesses potent emmenagogue (menstrual inducing) and uterotonic activities. It can induce uterine contractions, leading to miscarriage, premature labor, or fetal distress.

• Lactation: Safe usage has not been established. Active sesquiterpene lactones may be excreted into breast milk. Breastfeeding should be discontinued if therapy is essential.

Over Dosage:

• Symptoms: Extreme gastrointestinal upset, severe nausea, vomiting, prolonged diarrhea, and heightened allergic dermatological reactions.

• Management: There is no specific antidote. Treatment is supportive and symptomatic. Maintain hydration and electrolyte balance.
